This complaint form is only applicable to Allen County, Indiana.

Call (260) 449-7506 to File a Complaint of a Violation of the Allen County Smoking Ordinance.   

If you are filing a complaint within the city limits of Fort Wayne, Indiana,  please call (260) 427-7665.
